What LSVT BIG Is

LSVT BIG is a standardised physiotherapy programme developed specifically for people with Parkinson’s disease. Parkinson’s tends to reduce the size of movement over time — steps get shorter, reaching gets smaller, everyday actions shrink without the person noticing. LSVT BIG trains the opposite: larger, more purposeful movement, practised until it starts to feel normal again.

It is one of the more researched physiotherapy protocols for Parkinson’s, and it is delivered only by clinicians certified in the method.

Delivered by a Certified Clinician in Edmonton

Giri Srinivasan, Registered Physiotherapist and clinic owner at In Step Physical Therapy, is a certified LSVT BIG clinician. The full protocol is delivered at our Edmonton clinic.

How the Programme Runs

LSVT BIG follows a set structure rather than a loose plan:

  • Four sessions a week for four weeks — 16 one-to-one sessions in total
  • Daily home practice on treatment days and non-treatment days
  • One-to-one delivery throughout, not group-based
  • Carryover work tied to the activities that matter to you

The intensity is deliberate and it is a real commitment. We discuss whether it fits your circumstances before starting.

What It Works On

  • movement amplitude — bigger steps, fuller reaching
  • walking speed and stride length
  • balance and turning
  • rising from a chair, dressing, turning in bed
  • confidence in everyday movement

Individual response varies. Assessment determines whether LSVT BIG is appropriate for you, and we will say so if another approach would suit you better.

LSVT BIG and LSVT LOUD

LSVT BIG addresses movement. Its companion programme, LSVT LOUD, addresses voice and speech volume and is delivered by a speech-language pathologist. The two are separate programmes and some people benefit from both.

See our speech-language pathology page for the voice and communication side.

Is It Right for You?

LSVT BIG is designed for people with Parkinson’s disease. It is most often used in the early and middle stages, though suitability is individual and determined at assessment rather than by stage alone.

Your physician remains central to managing your Parkinson’s. LSVT BIG sits alongside that care rather than replacing any part of it.

See also our Parkinson’s rehabilitation and neurological physiotherapy pages.

Frequently Asked Questions

The standard protocol is four sessions a week for four weeks, sixteen one-to-one sessions, with daily home practice alongside. The structure is fixed because that intensity is part of what the programme relies on.

No referral is required to see a physiotherapist in Alberta. Some insurance plans request one before reimbursing, so it is worth checking your plan.

It is often used in earlier and middle stages, but suitability is individual and determined at assessment rather than by stage alone.

LSVT BIG addresses movement and is delivered by a physiotherapist. LSVT LOUD addresses voice and speech and is delivered by a speech-language pathologist. They are separate programmes and some people do both.

The programme is delivered one-to-one at the clinic. Home practice between sessions is part of the protocol and we will show you exactly what to do.

Many extended health plans include physiotherapy. Coverage for a sixteen-session programme varies by plan, so we recommend checking your limits before starting. We can explain what applies.

★★★★★
I’ve been with InStep Physical Therapy since February 2026, and their highly skilled team has been pivotal in my rehab journey. I’m recovering from a severe neurological nerve injury, and from my first interaction, the team has been as invested in my progress as I am in myself. They equip me with the educational tools I need, apply a tailored, data-driven approach to my care, and guide me consistently as I work through this sustained recovery. I am so grateful!

I went to InStep one year ago for bell's palsy that had come on suddenly without warning paralyzingly half my face. I was scared and unsure what to do. Giri worked with me patiently for weeks, each week with his help I made a slow but complete recovery. They helped me through one of the most difficult times of my life and I am very grateful

I have RRMS, and had no idea that physiotherapy could help me. I was having trouble with my hips, legs and back, so researched neuro pt after a nurse practitioner suggested it was worth a try. In Step clinic popped up in my online search repeatedly. I started working with Muthu, and in 3 short weeks, the improvement has been marked. He is a great educator, explaining what muscles we are focusing on and why. He has provided an excellent home stretching and exercise program that I complete everyday. The clinic is lovely and welcoming - friendly staff. The facility is clean and well managed, with appointments happening on time. The ask Giri physio show has taught me about MS and neuro pt, and I plan to watch every episode. Five + stars, and a big thanks!!
