Pelvic Floor Physiotherapy for Prostate-Related Concerns
The prostate sits directly above the pelvic floor, and prostate conditions and their treatment frequently affect how those muscles work. Whether you are managing symptoms from an enlarged prostate, recovering from surgery, or dealing with ongoing pelvic pain, pelvic floor physiotherapy addresses the muscular side of that picture.
This is men’s health physiotherapy delivered one-to-one in a private treatment room at our Edmonton clinic.
Rehabilitation After Prostate Surgery
Urinary leakage after a radical prostatectomy is common, and for most men it improves over the months that follow. Pelvic floor muscle training is the most established physiotherapy approach for supporting that recovery, and research generally supports starting it early.
What rehabilitation involves:
- Assessment of pelvic floor function — establishing whether you can contract, hold and fully relax those muscles, since many men are doing the exercise incorrectly without knowing.
- A progressive training programme — built around your starting point rather than a generic handout.
- Functional training — coordinating the pelvic floor with standing, lifting, coughing and the movements where leakage actually happens.
- Bladder habits and fluid advice — practical adjustments that often make a noticeable difference.
- Return to activity — graded return to walking, the gym and sport.
Ideally, start before surgery. Where there is time between diagnosis and your operation, learning to find and train the pelvic floor beforehand puts you in a better position afterwards. If surgery has already happened, it is not too late to begin.
Other Prostate-Related Concerns
- Benign prostatic hyperplasia (BPH) — where urinary urgency, frequency or incomplete emptying are affecting daily life, bladder retraining and pelvic floor work may help manage symptoms alongside your medical care.
- Chronic prostatitis and chronic pelvic pain syndrome — often involves pelvic floor muscle tension rather than infection. Treatment focuses on downtraining those muscles, manual therapy and pain management.
- Post-radiation changes — where radiotherapy has affected bladder or bowel function.
- Erectile dysfunction — frequently overlaps with prostate treatment. See our erectile dysfunction page.
Your Medical Team Comes First
Prostate conditions are diagnosed and managed by your physician, urologist and oncology team. Physiotherapy addresses the pelvic floor and functional side of recovery; it does not treat the prostate condition itself and does not replace any part of your medical care.
Speak to your physician about new or changing urinary symptoms, blood in the urine, or any symptom that is worsening rather than settling. If something needs medical review, we will tell you.
What to Expect at Your First Visit
The first appointment is a conversation before it is anything else. Your physiotherapist will ask about your symptoms, your surgery or treatment, and what you want to get back to.
Assessment is explained fully before anything is done, and nothing proceeds without your consent. Internal assessment is sometimes appropriate but is never assumed and never required — there is useful work to be done either way. All appointments are one-to-one in a private treatment room.
Recovery timelines vary considerably between men. We review progress as we go rather than promising a schedule.

Frequently Asked Questions
How soon after prostate surgery can I start physiotherapy?
That depends on your surgeon guidance and your catheter removal. Many men begin gentle pelvic floor work soon after, and where possible starting before surgery is better still. Contact us and we will discuss timing.
Will the leakage go away?
Most men see improvement over the months following surgery, though the pace varies and some continue to have symptoms. Pelvic floor training is the most established approach for supporting that recovery. We will be straightforward with you about what we are seeing rather than promising an outcome.
Is an internal examination required?
No. Internal assessment is sometimes appropriate and can give useful information, but it is never assumed and never required. Everything is explained first and nothing proceeds without your consent.
I was given a sheet of pelvic floor exercises. Is that not enough?
It may be, but a large proportion of men perform the contraction incorrectly, often bracing the abdomen or holding the breath instead. Assessment establishes whether you are activating the right muscles before building a programme on top.
Do I need a referral?
No referral is needed to see a physiotherapist in Alberta. Some extended health plans ask for one before reimbursing, so it is worth checking your plan.
Can physiotherapy treat my prostate condition?
No. Your prostate condition is managed by your physician, urologist or oncology team. Physiotherapy addresses the pelvic floor and functional side of recovery alongside that care.
